Congestive Heart Failure (CHF) is a condition in which the heart's function as a pump is inadequate to meet the body's needs. When the heart's pumping action is weak, the blood "backs up" (becomes congested) behind the heart. This congestion can lead to fluid accumulation in the lungs, kidneys, liver and body tissues. In the United States, the most common causes of CHF are coronary artery disease, high blood pressure. alcohol abuse, and disorders of the heart valves. CHF affects nearly 5 million Americans and is the most common cause of emergency room visits and hospitalizations among the elderly.
Although CHF is a serious condition, with proper medical care, medication, diet and exercise, symptoms and complications can be successfully treated and managed.
Working with your doctor, your CMO Nurse Care Manager will:
- Help review your current health status and your medicines
- Provide you with information about your condition
- Put together a personal treatment plan
- Help you learn and use self-care and self-monitoring tools
- Teach you how to monitor your health and take care of yourself
- Help you recognize possible complications
- Provide you with tips and information about life-style choices
- Call you to help keep your treatment plan on track
- Follow-up after you come home from the hospital
- Tell you about support groups and workshops
